Continuing Care Program and Services to Help You Live Well at Home

October is Continuing Care Month in Nova Scotia, a time to recognize the dedication and compassion of the Continuing Care Assistants, nurses, support staff and volunteers who help Nova Scotians live safely and independently. 

Continuing Care supports people at home and in their communities by working with individuals and their caregivers to understand their needs, explore available options and develop a plan of care. 

A range of programs and services is available, including: 

  • Home Support Services to assist with personal care, meal preparation, respite and essential housekeeping. 
  • Home Support Direct Funding to help eligible individuals pay for private home support services. 
  • Nursing Services at Home for services such as wound care, catheter care, IV therapy and other nursing needs. 
  • Caregiver Benefit Program for eligible people providing care to someone with a low income and high care needs. 
  • Family Relief/Respite Services to give caregivers a short-term break, either at home or in a facility. 
  • Equipment loan programs, including hospital beds, wheelchairs and mechanical lifts. 
  • Home Oxygen Services for eligible individuals who require oxygen equipment and supplies at home. 
  • Personal Alert Assistance Program to help eligible people access personal alert devices or emergency response services. 
  • Transition to Long-Term Care for people whose needs can no longer be met through care at home. 

Anyone can make a referral as long as the person being referred is aware. Simply call 1-800-225-7225 to discuss your needs and learn what support may be available.
